Opt Out of CarmenBooks Inclusive Access Form

This form is no longer active. Opt-outs must be submitted from your CarmenBooks bookshelf within CarmenCanvas. 

During the opt-out period before the second Friday of any session, you will see an opt-out button next to the material(s) for each course. Selecting to opt out will remove all titles for the course. Screenshot below.

Enter the 8- or 9-digit number from your BuckID.
Course Information
Enter the department or subject code followed by the course number as it appears in BuckeyeLink.
Enter the course name as it appears in BuckeyeLink.
Enter the class number as it appears in BuckeyeLink.
By completing this form, I confirm that I am opting out of receiving the automatic delivery of reduced-cost textbooks and online access to course-related materials. I understand that by opting out of the fee that I will not be entitled to the automatic delivery of the associated textbook and/or online access to other course-related materials at the negotiated discount rate that other students will receive. I fully accept the responsibility to legally obtain these items on my own and understand that failure to do so may impede my ability to stay current in class and may affect my academic success.
